Notice of Privacy Practices

Your child's health information is protected by federal law. Here's what that means for your family.

Thompson Pediatric Dentistry is required by law (HIPAA) to maintain the privacy of your child's protected health information, to give you notice of our legal duties and privacy practices, and to follow the notice currently in effect.

How health information may be used

We use and disclose health information for treatment (for example, coordinating care with another provider or a hospital for a sedation case), payment (filing claims with TennCare or your dental plan and verifying benefits), and healthcare operations (running a safe, high-quality practice). Other uses and disclosures generally require your written authorization, which you may revoke at any time.

Your rights

  • Request a copy of your child's dental records, and ask us to correct information you believe is wrong.
  • Ask us to communicate with you in a specific way (for example, calling only your mobile number).
  • Request limits on what we share, and receive a list of certain disclosures we have made.
  • Receive a paper copy of the full Notice of Privacy Practices at any time, and file a complaint — with us or with the U.S. Department of Health and Human Services — without any effect on your child's care.

The full notice

This page is a plain-language summary. The complete Notice of Privacy Practices is available at our front desk — ask any time, or call (615) 349-8450 and we'll mail or email you a copy. Privacy questions can be directed to the practice at smiles@thompsonpd.com.
